This position is only for VA Maine Healthcare full-time and part-time employees.
This position is a full time position.
Tour of Duty: 7:30 a.m. - 4:00 p.m.

Salary is determined by a Nurse Professional Standards Board computed by education and experience.
The Geriatric Clinic Care Coordinator is a professional who coordinates care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. The coordinator works closely with the primary care provider(s) and other healthcare professionals and team members, other clinics, internal and external services and community agencies.
The Geriatric Clinic Care Coordinator interprets the philosophy and goals of Nursing to others in the work setting; functions within established guidelines and demonstrates the ability to work effectively and develop sound relationships with patients, their caregivers and other professional and supportive personnel who provide patient care.

DUTIES:
The Geriatric Clinic Care Coordinator provides professional assessments, coordination and planning of multiple health care services; acts on behalf of the Veteran to assure that necessary clinical services are received and that progress is being made. In addition, the Care Coordinator provides ongoing evaluation of care management services.
He/she visits Veteran's who are in contracted nursing homes in the community ensuring optimal clinical care of those patients and assessing them and their families.
The Geriatric Clinic Care Coordinator will oversee the admission process for Veterans applying for admission to the Community Living Center.
Clinical Practice:
Provides initial and ongoing assessment of patients to identify needs issues, care goals and appropriate resources necessary for care management. Provides leadership in application of the nursing process and identifies resources and critical factors for achieving desired outcomes for health maintenance/improvement and post hospitalization recovery when appropriate. Sets clinical care goals, short and long term, in collaboration with patient, provider(s), and family members. Functions as a system coordinator for the plan of care; intervenes as appropriate to facilitate achieving patient outcomes within anticipated timeframes. Coordinates care with the patient's primary care provider and team. Collaborates with patient and care providers in any and all settings where care is being provided to evaluate and update changes in the therapeutic plan of care and patient management. Recognizes complex situations that impact patient care and intervenes, using sound judgement, professional attitude and appropriate channels.
Admissions Coordinator:
In collaboration with the Interdisciplinary Team on the Community Living Center (CLC) units, the RN evaluates all requests for admission and makes clinical decisions related to appropriateness of admission. The Registered nurse must consider space available, staffing levels and competencies of staff when making admission determinations. Must also work collaboratively with other team members across the organization. Communicates with nursing staff on the Community Living Center (CLC), and other medical units in the hospital and with outside facilities to ensure smooth flow and transition of Veterans across the continuum.
Contract Nursing Home Oversight:
Actively participates in Contract Nursing Hone (CNH) oversight committee as a key member.
Visits all contract residents according to the CNH handbook guidelines and documents visits in CPRS.
Reports all areas of concern to the CNH Coordinator.
QUALIFICATIONS REQUIRED:
REQUIRED:
Citizen of the United States.
Must be proficient in spoken and written English.
Current, full, active and unrestricted nursing license from a State, Territory or Commonwealth of the U.S.
Baccalaureate degree in nursing from NLNAC or CCNE accredited program and approximately 2-3 years of successful clinical nursing practice (RNs appointed before 12/10/99 must meet interim education requirement of Associate Degree or diploma in nursing from NLNAC or CCNE accredited program)
Demonstrated ability in the areas of interpersonal relations, critical thinking, problem solving, and conflict resolution.
Master's degree in nursing from NLNAC or CCNE accredited program and approximately 1-2 years successful nursing practice.
Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization, Professional Development: performance, education/career development, Collaboration; collaboration, collegiality, Scientific Inquiry: quality of care, research. Education/Experience Requirements:
- Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
- Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
- Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
- Nurse II - A BSN with approximately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
- Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.

Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia . In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. Must be proficient in spoken and written English .
