JOB SUMMARY:
Works under the direct supervision of the Regional Manager and Director Care Coordination in an expanded! nursing role to coordinate and provide disease management education
and clinical management for Beacon Health, LLC Plan members enrolled in Health Management programs. Collaborates with primary care providers in multiple practice sites to facilitate clinical guideline adherence and coordinate services specifically related to the members’ health care
needs. Integrates identified member needs, education pathways, clinical guidelines and
diagnostic tests to develop individualized plans of care.

Utilizes marketing skills in the Health Plans network of providers to identify and enroll members
with chronic health conditions in Health Management programs, as well as refer members to appropriate case management and specialty services. Assesses community and employer
work-site needs to support wellness and health management programs.

Collects and analyzes data, maintains documentation and adjusts interventions to maximize
clinical, qualit! y, and fiscal outcomes. Participates in Quality improvement ac! tivities as required.

Demonstrates strong interpersonal skills to interact in an effective manner with the practitioners,
the health care team, members and their families.

Demonstrates the ability to work autonomously, establish priorities, and be directly accountable
for practice.

Demonstrates the ability to be flexible, function effective in a fluid, dynamic, and rapidly
changing environment.

Demonstrates leadership qualities including time management, problem solving and
decision-making, priority setting, work organization, and verbal/written communication skills.

Demonstrates advance assessment and clinical skills with chronic illnesses to function as a
generalist with a managed care population.

Demonstrates the ability to analyze outcomes and design interventions.

PRIMARY RESPONSIBILITIES:

  • 1. Assists leadership in the design, implementation and evaluation of health ma! nagement
programs to support overall Beacon Health, LLC goals and strategic business strategies.

  • 2. Receives referrals for health management, assures appropriateness of member referrals,
and triages/stratifies members according to assessed needs for enrollment into Health
Management programs.

  • 3. Facilitates a positive teaching/learning environment that incorporates age appropriate
learning principles and supports Health Management’s education/clinical guidelines.

  • 4. Utilizes established education pathways to direct comprehensive and consistent disease management education to members. Assesses physical, social, and functional status to direct education/management activities.
  • 5. Coordinates services and serves as a resource to members, families, and staff for all Health Management programs.
  • 6. Collaborates with primary care providers to enhance evidence-based clinica! l guideline adherence and promote best practice by initiating/adjusting! therapies as directed by the
practitioner and providing appropriate follow-up and monitoring as needed.
Facilitates/coordinates laboratory and diagnostic tests for all disease management programs.

  • 7. Fosters a team approach by working collaboratively with the member, primary care physician, case manager, and other staff to ensure coordination of services, develop an individualized
plan of case, and promote responsible self-management.

  • 8. Continuously evaluates members’ progress toward desired health outcomes and adjusts plan of care for optimal outcomes.
  • 9. Markets the role and value of health management in disease management and case management to primary care providers, office staff and the community.
  • 10. Collects and documents data, analyzes and communicates results and documents as
required for Health Management programs.

  • 11. Maintains member confidentiality w! ith all activities in Health Management programs.
  • 12. Assesses health care needs for employer work groups and community sites, and provides appropriate health management programs. Collects, evaluates and documents interventions and outcomes for these programs according to health management standards. Works closely with onsite wellness team.
  • 13. Utilizes Health Management outcomes to promote benefits and availability of disease/case management services with providers and other health care providers.
  • 14. Identifies problems, proposes process improvement strategies and assists the Care Coordination team in Quality Improvement activities.
  • 15. Collaborates with other disciplines at practice sites to identify operational issues and Performance Improvement needs. Collaborates on action plan with health manager and clinic site supervisor, and follows through with interventions as indicated.
  • 16. Attends and participates in professional an! d nursing continuing education programs as needed to support the health! management programs and one’s professional development.
  • 17. Reviews the current literature regarding effective teaching/learning strategies and incorporates the appropriate techniques into their patient education sessions.
18. Incorporates excellent listening, verbal, written, and critical thinking skills in disease/health management interventions.

19. Provides support to other members of the Health Management team as needed.

20. Performs other duties as required or assigned by emergency or other operational reasons for which the employees are qualified to perform.

REQUIREMENTS:
Requires an RN license, or eligible for licensure in the state of Maine. Bachelor’s degree in nursing and/or health care preferred.

Minimum of three to five years’ experience in clinical nursing with emphasis on patient education and program management required. Ambulatory c! are/managed care experience is preferred.

Computer experience required.

Requires the ability to travel to multiple office locations and manage fluctuating work hours, including off shift, weekend, etc. as needed. Valid Maine driver’s license and willingness to travel is required.

EDUCATION and TRAINING:
Registered nurse licensed in the state of Maine required.
