The VA Maine Healthcare System is seeking a Social Worker (MFH Program Coordinator).
The Medical Foster Home Program is an important element in the continuum of care for Maine Veterans offering an alternative to nursing home institutional care by providing foster care in the home setting. This position oversees the establishment of new foster homes, facilitates the placement of veterans into these homes and provides ongoing monitoring to ensure the success of these homes .

DUTIES:
Incumbent serves as program coordinator and is administratively responsible for the Medical Foster Home (MFH) program providing treatment to veteran patients in this national VA program. S/he is a practitioner in this specialty providing direct patient care services in the program area. They manage the daily operation of the program, develop policies and procedures for program operation and prepare reports and statistics for facility, VISN, and national use. They are responsible to provide an accounting for the programÂ's budget. The incumbent manages and controls the use of data related to these programs regarding day to day functioning and program evaluation. In conjunction with the Service Chief, plans, organizes and directs administrative, operational, patient and personnel activities for the service which includes:
Administers information and analytical systems to evaluate and enhance the quality of service provided to patients. Develops and implements information systems to track service activities including visits, referrals, patient demographics, equipment utilization and patient satisfaction. Develops implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
The incumbent provides administrative, technical and clinical supervision necessary for accomplishing the work of the program.
Provides informal feedback to medical foster home staff and sponsors. Incumbent is able to resolve informal complaints and grievances. Incumbent provides advice and counsel to workers related to work and administrative matters.
The incumbent partners with and works collaboratively with MFH staff and the Home Based Primary Care team. S/he is responsible for frequent communication with the team through interdisciplinary meetings, staff meetings and clinical documentation.
The incumbent uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. The incumbent performs program analysis, development, review, research, design, and contact work as possible with various agencies (public, DOD, state, etc.).
Performs work related to the review, monitoring, analysis, planning and assessment of various programs. Responsible for key project activities related to the planning, development, and implementation of the project component. A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main unit under conditions that make day-to-day supervision difficult to administer.

QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
- Citizenship. Be a citizen of the United States.
- Education. Have a masterÂ's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
- Licensure. Must be licensed or certified by a state to independently practice social work at the masterÂ's degree level.
- English Language Proficiency. Social workers must be proficient in spoken and written English.
Experience, Education and Licensure. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level. Their experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. They may have certification or other post-masterÂ's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. In addition, the candidate must demonstrate the professional KSAs provided below.
Demonstrated Knowledge, Skills, and Abilities
1. In-depth knowledge of the program coordinated, and demonstrated knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
2. Knowledge and skill in management/administration, which includes supervision, consultation, negotiation, and monitoring.
3. Ability to supervise multidisciplinary staff assigned to the program.
4. Ability to organize work, set priorities, meet multiple deadlines, and evaluate assigned program area(s).
5. Ability to provide training, orientation, consultation and guidance within clinical specialization of practice.
