Candidates must be located within an hour commute of an Aetna office on the east coast to be considered for this telework position.POSITION SUMMARY
This is ! an exciting new opportunity providing case management services exclusively to our members with Congestive Heart Failure (CHF) under the age of 65.
The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license.
EDUCATION
The minimum level of education desired for candidates in this position is an Associate's degree or equivalent experience.REQUIRED LICENSURENursing/Registered Nurse (RN) is required
