K atahdin Valley Health Center is seeking a PMHNP for their Outpatient Community Health Center located in Houlton, Maine. Houlton is a historical service center with 15,589 people, complete with eateries, shops, friendly locals and a downtown market square which serves at the County Seat for Aroostook County.
With the mission of providing community accessible, quality healthcare with compassion and dignity, KVHC is the largest Federally Qualified Health Center organization in Northern Maine. KVHC offers integrated, comprehensive, and affordable healthcare for the entire family through six Patient Centered Medical Homes. Katahdin Valley Health Center is a National Committee for Quality Assurance Level 3 Patient Centered Medical Home (PCMH) as well as an accredited PCMH organization through the Accreditation Association for Ambulatory Health Care since January of 2014. The patient payor mix is comprised of 39% Private Insurance, 24% Medicaid, 20% Uninsured and 17% Medicare. System wide annual patient visits exceed, 37,000, served by a staff of 22 practitioners and 115 team members. KVHC services include Primary Care, Behavioral Health, Dental Care, Walk-in Care, Optometry, Occupational Medicine, Podiatry, Pharmacy, Immunizations and Screenings, Laboratory, Pediatrics, Woman's Health and Telepsychiatry. Each of KVHC's locations employ Patient Service Representatives, Care Management, Health Education and Eligibility Assistants to assist patients with community resources, eligibility, prescriptions, sliding fee and Health Care Exchange enrollment.
Responsibilities and Duties
Practitioners joining KVHC will provide outpatient community mental health services to KVHC patients of all ages on a Mon - Fri, 8:00am â€" 5:00pm schedule with no nights, weekends, call, or holiday coverage. The PMHNP will collaborate with the Primary Care Providers to recognize, treat, and manage mental health and psychosocial issues while acting as a contributing member to the integrated primary care team. This position requires knowledge, experience and active support for rural, community oriented care. KVHC offers state of the art facilities, technology, and utilizes Athena Health Electronic Health Records system.
Qualifications and Skills
Requirements/Qualifications:
Applicants must possess a Bachelor's Degree in Nursing followed by a Master's Degree in Psychiatric or Mental Health Nursing from an accredited school. Licensed as a Psychiatric Mental Health Nurse Practitioner in the State of Maine.
