GENERAL DESCRIPTION:
Provide high quality, team based infectious disease care to patients of all ages with HIV, hepatitis and latent TB. Serves in an expanded nursing role to collaborate with the Medi cal Team to provide a model of care that ensures the delivery of quality, efficient, and cost effective healthcare services. Integrates evidence based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare. Responsibilities include telephone and walk-in triage, direct patient care, patient education, population health management and care coordination.
Program Manager is responsible for developing, coordinating, and managing the Ryan White Part C program and providing best practices for HIV/AIDS treatment. Coordinates focused HIV/AIDS services to patients and families by performing the following duties: developing systems, procedures and protocols, participating in ongoing training, positively representing Greater Portland Health to the community, ensuring efficient, patient-focused work flows (including intake), processes, and practice management systems, providing clinical and administrative supervision, and program oversight and leadership
ESSENTIAL JOB FUNCTIONS: *
Direct Patient Care:
- Providing infectious disease and primary care services to a panel of diverse patients.
- Responsible for providing care in a sensitive and non-judgmental manner.
- Performs diagnoses, treatment and/or proper referral with special attention to infectious disease patients while maintaining expected medical standards.
- A good understanding of the psychosocial/medical needs of the client and basic care management principles in order to formulate an integrated approach to treatment that takes into consideration the complicated health problems of the client.
- Functions as an advocate for patients, particularly those with fewer economic and social resources.

- Monitor patient progress and promote early intervention and group decision making.
Care Coordination:
- Follows HIV, Hepatitis C and Latent TB practice guidelines; including preventative services, immunizations and chronic disease management.
- Proficient in the management of acute and chronic conditions, both simple and complex
- Will maintain and continue to enhance the knowledge base and updated approaches regarding HIV/AIDS treatment. This includes keeping abreast of relevant alternative treatment approaches.
- Manage utilization and practice metrics to refine the delivery of care model to maximize clinical, quality and fiscal outcomes for the population
- Provide safe transition of care across the continuum of care
- Implement monitoring systems for high-risk member to prevent and/or intervene early during acute exacerbations. Works with the primary care physician to ensur e appropriate standing orders for acute exacerbation management (such as diuretic titration protocol).
- Continuously evaluates laboratory results, diagnostic tests, utilization patterns and other metrics to monitor quality and efficiency results for assigned population.
- Collaborates with the Transitional Care Coordinators to ensure smooth process in place when movement between settings occurs
- Convenes daily team huddle and facilitates discussions regarding patient care and concerns for the day.
- Provides regular progress updates to PCP and other members of the care team.

EDUCATION, EXPERIENCE AND KNOWLEDGE QUALIFICATIONS:
- Minimum of two years of clinical experience with emphasis on HIV/AIDS patient care
- Experience in sexually transmitted infection screening, treatment, and education
- Registered Nurse with three to five years of recent (within the last 3years) acute care experience required.
- Licensed at an RN level by the State of Maine Board of Nursing.
- Bachelor's degree in Nursing is preferred.
- BLS Certification.
- Experience in care management/care coordination to include care planning, resource management, and utilization management desired.
- Prior experience in Primary Care setting desired.
SKILLS AND ABILITIES QUALIFICATIONS AND OTHER REQUIREMENTS:
- Knowledge of the principles and practice of infectious disease services and preventive medicine.
- Needs to be professional as well as knowledgeable, responsible, and accountable to manage patient care over the continuum and to assist in improving quality, cost, and patient/service satisfaction outcomes.
- Ability to perform tasks that require the skills of a registered nurse including patient education related to chronic condition care, assess patients in rare instances when a patient may walk-in to GPH and are in distress requiring a skilled assessment and discussion with the on-site provider regarding disposition.
- Will be responsible for the assessment, planning and implementation process for a select group of patients in the outpatient setting.
- Ability to perform the same function as Medical Assistant staff; rooming patients, immuni zations, procedures.
- Must be organized with the ability to self-motivate and prioritize a variety of duties.
- Ability to motivate others and work effectively with clinical and non-clinical staf
- Demonstrates excellent communication skills.
- Demonstrates the ability to perform multiple tasks simultaneously.
- Ability to work with people from diverse backgrounds and experiences
- Ability to openly address and acknowledge observed issues and concerns
- Attention to detail, and ability to be flexible in performing a variety of tasks.
- Demonstrated ability to complete projects within designated timelines and the ability to prioritize duties is required.
- Demonstrates proficiency with the electronic medical record (NextGen) Microsoft Word and Windows & Excel, and the ability to learn various system software
- Demonstrates proficiency in basic computer skills, including accurate keyboard entry of data into relevant computer based databases and spreadsheets.
- Demonstrates ability to use basic office equipment, computer, fax machine, copier, calculator, etc.
